What is an explanation of benefits?
- An explanation of benefits (EOB) is a document that is sent by your insurance provider that explains how your claim was processed.
- It indicates what was paid and/or what was denied along with basic account information such as allowed charges, co-pays, covered and non-covered tests, and explanation of payments.
- It is not a bill, nor does it represent final patient responsibility in some cases.
You do not need to pay anything until you receive a bill from us.
Why am I being billed?
Your insurance policy requires your responsibility for a co-pay or deductible charge.
- Your policy may not cover certain laboratory tests.
- Your policy has maximum benefit limitations.
- Information regarding your group number, policy number, diagnosis code, or other required information to process your claim was not provided to Luminus Diagnostics.
- Your insurance company needs information from you as stated on your explanation of benefits (EOB).
- We were instructed by your physician to bill you directly.

- If you receive payment directly from your insurance company for Luminus Diagnostics testing, please endorse check with your name and the statement “Endorsed over to Luminus Diagnostics LLC” along with your EOB.
